Skill Terminal Competency – displays student’s “batting average” for each skill – red indicates below passing, green indicates terminally competent

Patient Contacts – displays students’ cumulative patient contacts

In-Class Participation and Student Affect – use this form to submit an in -class participation and affective evaluation on a student

Field Evaluation and Team Leads Report – displays sum of all field evaluations, total team leads

Formative Evaluation – displays attempts during skill development phase, which students needs remediation, which were successful, and which still need to be evaluated

Clinical Evaluation Request – displays the number of requests each student should have submitted, how many they actually did submit, and how many responses from preceptors were received
